Upstate New York Nursing Home Could Have Potentially Prevented Abuse

The nursing home also failed to provide abuse prevention training and to properly investigate potential cases of abuse, neglect, and mistreatment.

According to the nursing home was cited for 24 deficiencies in 2015 and 2016. The centers for Medicare and Medicaid services gave the facility its lowest rating, one star (meaning much below average).
